Overview
This one-day course will bring together the research and integrated care teams to discuss the latest developments around transitional care. The course faculty consists of renowned physicians, social workers, nurses, and other allied health providers from Northwestern and from around the United States. This course will provide providers at all levels with the knowledge and insight into particular challenges in caring for high-use patients. Topics include innovative models of care transitions, addressing social determinants of health, hospital-based programs for high utilizes, and a review of literature and public policy surrounding transitional care. 

Objectives
Explain current and future challenges and opportunities for transitional care.
Cite current evidence in transitional care and identify areas for future investigation.
Identify key community partnerships integral to transitional care.
Summarize current bridges between hospital and community initiatives in transitional care.
Describe the interface between transitional care evidence, policy, and public perspective.
Identify practice and policy challenges to transitional care and lead effort to support patients in care transitions.
Compare current practices to optimize patient centered care, communication, and expertise within the realm of transitional care.
