Title: Promoting Medical Testing Among the Wishful
Abstract: People frequently avoid medical testing even when it is free and highly valuable. This problem is especially critical for poor households in developing countries, where vulnerability to illness is greatest yet demand for health information remains far below optimal levels. We propose and evaluate a coupled testing mechanism in Uttar Pradesh, India. Leveraging spousal altruism, this design induces higher testing uptake than standard individual offers: a spouse who fears their partner may be ill has a strong incentive to consent, effectively counteracting the other's avoidance. We explore the range of behavioral mechanisms through which the intervention may operate.
